What should I bring with me to my first appointment?

If possible, you should bring picture identification, insurance card, completed new patient paperwork (available on this site), a list of medications and your co-pay if applicable. You should also bring proof of income if you wish to be considered for the sliding fee scale.

What kind of proof of income is acceptable?

For every working adult in the household, you should provide one of the following items to show proof of income: A current tax return or W-2, a month’s worth of current check stubs, a social security letter, or public assistance letter such as SNAP benefits. If you have unique circumstances or need assistance, please call our billing department at (406) 496-6000 and press 0.

What type of insurance does the clinic accept?

SWMTCHC accepts all insurances, including Medicare and Medicaid. However, we urge insured patients to check with their specific plans for details about networks and coverage.

Does the clinic accept all patients?

The clinic accepts everyone, regardless of ability to pay. It accepts insured and uninsured alike.

What if I can’t afford my medication?

If you find that you can’t afford a prescribed medication, we ask that you contact care management at the clinic for their assistance in exploring ways to help. They can be reached at (406) 723-4075.

If I am homeless will the clinic serve me?

The clinic has a special program that assists homeless people who need access to primary care. We also provide outreach at offsite clinics at the Butte Rescue Mission and Emma Park. If you are homeless, please call a care manager at (406) 723-4075 for assistance.

What if I have a painful toothache. What can I do?

The dental clinic, located on the north side of the building in Butte, takes dental walk-in emergencies each weekday morning beginning at 7:45 a.m. on a first-come, first-serve basis. The exception is the second and fourth Fridays of the month which are reserved for children. If you need more information call the dental clinic at (406) 496-6007.

I’m sick. How can I get in fast?

The Butte clinic offers same-day appointments every day, including Saturday and Sunday. Please call (406) 723-4075 for help.

What ages do you treat?

We offer services to patients of all ages from birth to the senior population. We offer primary medical, dental, behavioral health, pharmacy and care management to all.
