Frequently Asked Questions

We offer several different ways to help you find the information you need. Please use the links below to aid you in your search for the topics that are of interest to you. If you cannot find the answers to your question with the sections below, please contact us with your question.

“What services do you provide?”

The Southwest Montana Community Health Center offers primary medical care, dental, behavioral health, substance use, care management, and pharmacy.

“When are you open?”

Butte Medical Clinic is open Monday through Friday 8:00am to 7:00pm and Saturday from 8:00am to 5:00pm, closed 12-1 on Thursday.
Butte Dental Clinic is open Monday through Friday 8:00am to 5:00pm, closed 12-1 on Thursday.
Butte Pharmacy is open Monday through Friday 9:00am to 6:00pm and Saturday 9:00am through 5:00pm, closed 12-1 on Thursday.
Dillon Medical is open Monday through Friday 8:00am to 5:00pm, closed for lunch hour. **(sometimes it is 1:00 and sometimes 12)
Dillon Dental is open Monday through Friday 8:00am to 5:00pm for appointment scheduling, and dental care Tuesday & Thursday 8:00am to 5:00pm.
Mac’s Pharmacy located in Sheridan is open Monday through Friday 9:00am – 5:30pm.

“Am I eligible for a discount?”

If your household income is at 200% of the Federal Poverty Level (link to guidelines on About/Discounts page), then you’ll be eligible for a discount. If it is 100% or less, you only need to pay a nominal fee for your visit of $20 for Medical/Behavioral Health and $40 for Dental.

“How do I get a discount on my prescription medicine from CHC pharmacy?”

Once your proof of income is on file at the clinic and if you are < 200% of FPL, the pharmacy can access the information and provide the discounted price on the medication.

“How do I show that I am below 200% of the Federal Poverty Level?” 

You can bring in proof of income when you come for your visit. The best form of proof is a copy of your latest income tax return. Other types of proof include pay stubs, W-2s, Social Security documents, 1099s, etc. Remember that we need all household income and how many people live in the household.

“What if I didn’t bring proof of income?” 

If you didn’t bring your proof of income, you can complete and sign a self-declaration. This will be good for 30 days. After 30 days, we will need to provide proof of income to continue your discount.

“What if I have insurance or I am over 200% of poverty?”

The Southwest Montana Community Health Center welcomes all patients. We accept all insurance, whether you are above or below 200% of poverty. While we don’t give discounts to those over 200% of poverty, we will still treat you and let you pay your balance over time. If you qualify for a discount and have insurance, the discount will be applied after your insurance pays, which can be especially helpful for those with high deductibles.

“How do I get a copy of my medical records?” 

You need to come into the clinic and fill out a release of information form or call and have us mail or fax a form to the patient. It will take up to 10 business days to get the copy of your record and you will be charged $0.50 per page.
